India's eSanjeevani hub-and-spoke telemedicine reached 8 crore rural consultations

India's free telemedicine links village centres to specialist hubs and homes to doctors — 8 crore consultations made it the largest government service.

The solution

India's specialist doctors are concentrated in cities, leaving rural patients long journeys and lost wages for a ten-minute consultation. The Ministry of Health's eSanjeevani was built to change the geometry of that problem.

Its doctor-to-doctor vertical uses a hub-and-spoke model: Ayushman Bharat Health and Wellness Centres at state level act as spokes, mapped to hubs of MBBS and specialist doctors at zonal level. A village health worker runs the video link, so the patient needs neither a device nor digital skills.

Its patient-to-doctor vertical, rolled out on 13 April 2020 as clinics shut in the first lockdown, lets anyone consult a doctor from home via smartphone or computer. C-DAC Mohali built and operates the platform end to end.

By December 2022 the service had clocked 8 crore teleconsultations — the last crore in about five weeks — across 1,09,748 Health and Wellness Centres and 14,188 hubs, with 1,144 online OPDs and 2,22,026 doctors and health workers onboarded, making it the world's largest government-owned telemedicine platform in under three years.

Why it worked

Aftermath

After 8 crore consultations by December 2022, the service kept compounding through 2023 and beyond. eSanjeevani is now part of the Ayushman Bharat Digital Health Mission, generating ABHA health IDs through its app, and it remains the reference point for how a developing health system scales scarce specialists to rural patients.
