Mastercard turned Germany's bank cards into legal organ donor registrations
Germany's donor consent was buried in paperwork, so Mastercard embedded it on the debit cards people already carry; about 50% of targeted users adopted it.
Mastercard
the move
Germany's organ donor shortage is a paperwork problem: 85% of the public supports donation, but only 15.3% officially document consent, and family refusal rates run at 75% when wishes are unclear. Digital registries drew barely 0.2% uptake because of privacy fears.
Mastercard's insight was that 86% of Germans carry bank cards daily — a trusted object already used for important decisions. It embedded a legally binding donor declaration into the cards, activated via the VIMpay app, with a red Life Donor heart marking the commitment.
The campaign paired the product change with trusted medical influencers and emotional stories, such as a triathlete competing after a heart transplant, turning the card itself into a constant reminder and a conversation starter.
why it works
- Bank cards are carried daily, so the medium never leaves the decision-maker.
- Signing a card is a familiar, low-friction act versus a bureaucratic form.
- Privacy concerns were answered with the infrastructure people already trust.
- A visible red heart turned every payment into an awareness moment.
what transfers
When a desirable behavior is buried in bureaucracy, find the object people already touch daily and put the decision on it; a routine financial act can carry a lifesaving choice.
what came after
Roughly half of targeted users adopted the card on issuance, over 20% of VIMpay customers immediately, and 44.5% said they would switch banks for the feature. With about 178 million German bank cards as a potential base, the model scaled documented consent and reached the Cannes Lions 2025 Titanium shortlist, later winning a Silver in Brand Experience & Activation and Bronzes in Direct and Creative Data, plus a One Show Bronze Pencil.
