E.164 gave every phone number one format: country code plus national number
The ITU plan splits any number into a one-to-three-digit country code plus national number, capped at 15 digits, so any phone reaches any other.

the move
Telephone networks grew country by country, each with its own numbering ideas, so international calls needed operators and translation between incompatible plans. As direct dialing spread, the world needed one address scheme.
The ITU's E.163 plan began that structure in 1964, and E.164, first issued in 1988 for the ISDN era, became the definitive international public telecommunication numbering plan, according to the ITU's own record. It defines the format for numbers on the worldwide public switched telephone network.
The design is a single tree: a one-to-three-digit country code allocated by ITU-T, then a national number of up to fourteen digits, with fifteen digits total as the ceiling. Every national plan fits inside the same structure, so direct dialing, roaming, and later VoIP addressing all route on the same prefixes.
