In 1994 the Chickasaw Nation compacted with the U.S. Indian Health Service to run its own 13-county health system in southern Oklahoma, taking over administration and finance while remaining under federal oversight. Every patient is served at no charge — the system cannot bill its users, and its benchmark Medicare spend sits at about 0.8 the national average — so the classic ways to raise money (charge more, sign up paying customers) are structurally closed.

The default answer for underfunded free-care systems is to ration, cut services, or fight for a larger allocation. None of those grows the pot; they only divide a fixed subsidy more thinly.

CNDH pays insurance premiums out of its own pocket for its own patients, instead of serving them as government-sponsored charity cases. Patient Benefit Coordinators enroll patients in third-party insurance, the department funds the premium payments for more than 1,600 patients, and every office visit and admission then triggers a payer reimbursement that flows back into the system — which reinvests it in nutrition, clean water and cash reserves, while the patient is still never charged.

The department pays insurance premiums for its patients, flipping each visit from pure cost into payer revenue that funds care, while patients are still never charged.

Paying premiums makes each patient an insured beneficiary, so every visit triggers a reimbursement the system can keep.

Reimbursements flow back into the system, compounding cash reserves instead of leaking as pure cost.

The payer's quality standards enforce external audit, keeping the system accountable and high-performing.

If you can't charge users, enroll them in a payer scheme yourself and bill against it, converting fixed cost into recurring revenue.

CNDH won the 2024 Baldrige National Quality Award and held CMS five-star top-10% ratings for six straight years.
