Rwanda stopped trying to store blood everywhere and started firing it out of a catapult instead
the problem
A perishable medical supply needed unpredictably and urgently at many remote locations forces an impossible choice: stockpile locally everywhere (and watch units expire unused before they're needed) or centralize and rely on road delivery (which is too slow and unreliable over difficult terrain for genuine emergencies)

Blood is a perishable resource with a short usable shelf life, needed unpredictably but urgently — a maternal hemorrhage or a trauma case can't wait for a multi-hour drive over Rwanda's mountainous terrain and unreliable rural roads. Stockpiling blood locally at every rural clinic to guarantee availability means units routinely expire unused at facilities that didn't happen to need that type that week, while relying purely on centralized storage and road-based restocking means clinics facing a genuine emergency simply don't have blood on hand when the drive time exceeds what a patient can survive.
Starting in 2016, the Government of Rwanda partnered with Zipline to centralize blood storage entirely rather than distribute it locally, and deliver it on-demand by fixed-wing drone — launched by catapult from a central distribution hub and dropped by parachute directly at the requesting clinic, with the drone returning to base rather than landing. The system meant no rural facility needed to carry its own blood stock at all; every request for a specific blood product could instead be filled centrally within minutes of being called in.
the move
Rather than optimize the tradeoff between local stockpiling (waste) and centralized road delivery (unreliable speed), the drone system eliminated the tradeoff structurally — centralization solved the waste problem completely, and drone delivery solved the speed problem over terrain where roads themselves were the bottleneck, letting Rwanda get both benefits simultaneously rather than compromising between them.
the payoff
A 2022 study published in The Lancet Global Health found the drone-delivery system reduced blood product wastage by roughly seven units per facility per month and substantially improved delivery responsiveness compared to road-based transport; by June 2022 Zipline had delivered more than 105,000 units of blood to 410 health facilities across 27 districts in Rwanda, marking the first large-scale use of drones for primary health-commodity delivery in Africa.
what came after
Zipline's Rwanda program is a standard case study in global-health logistics and humanitarian-technology literature for solving a last-mile perishable-supply problem by restructuring the underlying storage-versus-transport tradeoff rather than incrementally improving either side of it, and the model has since expanded into vaccine, medicine and other medical-supply delivery across multiple countries.
