Severe childhood pneumonia is one of the leading causes of death for children in poor countries, and the standard respiratory-support device that reliably saves these children — a mechanical ventilator — costs up to $15,000. District hospitals in Bangladesh, the secondary-level facilities that see the largest volume of severe pneumonia cases, had no realistic path to owning enough ventilators at that price, so children who failed to improve on basic low-flow oxygen had no further respiratory support available and often died.

The obvious response — get more ventilators into poor hospitals — was never going to happen at a $15,000 unit cost against district-hospital budgets, and there was no cheaper commercial alternative on the market delivering the same clinical function. Dr. Mohammod Jobayer Chisti, a senior scientist at Bangladesh's icddr,b, needed a device that could deliver the same physiological effect as a ventilator's respiratory support without requiring anything close to a ventilator's price.
