When the Great Baltimore Fire broke out on 7 February 1904, it quickly outgrew what the city's own fire department could contain, and mutual aid crews rushed in by train from Washington, Philadelphia, New York and other cities — exactly the redundant capacity a well-run mutual aid network is supposed to provide. Every American city fire department of the era had independently developed its own hose coupling thread pattern, diameter and pitch, with no coordination between cities that had never expected to need each other's equipment on the same hydrant.

The obvious response to a fire that burned for 30 hours and destroyed roughly 1,500 buildings was to conclude Baltimore needed more firefighters, more hoses, more water pressure — the visible resources that had clearly been insufficient. The actual bottleneck was invisible until crews arrived: engines and hose from Washington, Philadelphia and New York physically could not connect to Baltimore's hydrants at all, leaving fully staffed, fully equipped mutual aid crews standing uselessly beside a burning city.

The NFPA formed a committee within months of the fire and, drawing on new nationwide data on how many incompatible coupling variants were actually in use, published its first recommended fire hose coupling standard in May 1905 — a specific thread count, diameter and pitch (National Standard Thread) that any city could adopt to make its hydrants and hose interoperable with any other adopting city's mutual-aid equipment.

The standard was rapidly endorsed by multiple fire-service organizations, but adoption on the ground was strikingly slow and incomplete: by 1914 only 287 of roughly 8,000 US cities and towns had conforming couplings, and a 2004 NIST survey found only 18 of the 48 most populous US cities used the national standard — San Francisco's own incompatible hydrants were still flagged as a mutual-aid liability as recently as 2017.

National Standard Thread remains the most widely used fire hose coupling standard in the US today and the underlying diagnosis — that mutual aid depends on a literal physical interface standard, not just cooperation — is now foundational to emergency-response planning, but the century of uneven adoption that followed is itself now cited as a case study in how a correct technical fix can still fail to fully spread without an enforcement mechanism behind it.
