FDA cut birth defects by fortifying flour instead of asking women to take vitamins early
the problem
Folic acid could prevent neural tube birth defects, but the defects form before most women even know they're pregnant

By the early 1990s, researchers had firmly established that adequate folic acid intake around conception sharply cut the risk of neural tube defects like spina bifida and anencephaly, defects that struck roughly 4,000 US pregnancies a year. In 1992 the Public Health Service duly recommended that all women of childbearing age take 400 micrograms of folic acid daily. But neural tube defects form in the first three to four weeks of pregnancy, often before a woman knows she has conceived, and most US pregnancies were unplanned — so a recommendation depending on a woman starting a daily supplement before she had any reason to think she was pregnant was, structurally, always going to arrive too late for a large share of the people it was meant to protect.
Public health campaigns urging women to take a daily vitamin ran into exactly that timing problem no amount of better messaging could fix: you cannot ask someone to act on information she doesn't yet have a reason to seek out. The FDA needed folic acid to reach women who weren't planning a pregnancy and weren't taking any supplement at all, without asking any of them to change a single decision.

Run public health campaigns urging all women of childbearing age to take a daily folic acid supplement — a plan that only protects the subset who plan their pregnancy far enough in advance to start supplementing before conception, missing everyone else by design.
what they saw
The real obstacle wasn't ignorance of folic acid — it was timing: defects form before pregnancy is known, so any ask-first fix arrives too late. Fortifying food removes the decision point entirely.
the move
In 1996 the FDA mandated that folic acid be added to all enriched cereal grain products — bread, pasta, rice, flour — sold in the United States, with compliance required by January 1998. Because nearly everyone eats fortified grain regularly regardless of pregnancy plans, the intervention delivered protective folic acid to women of childbearing age automatically, months or years before conception, without requiring anyone to know they needed it.
why it works
It works because it decouples protection from awareness and intent — you don't need a woman to know she's pregnant, plan to be, or take a supplement, because the folic acid is already in the bread and pasta she was going to eat anyway. That also makes adherence universal rather than a matter of individual compliance, which is the biggest reason voluntary supplementation campaigns had underperformed for years beforehand.
the payoff
Within a year, blood folate levels nearly tripled, and neural tube defect rates fell an estimated 78 percent by 2001.
where it breaks
It only reaches populations whose diet includes the fortified staple, so it does little for people eating unfortified grain, low-carb, or gluten-free diets outside the mandate's reach, and it can't be targeted to only those who need it, so it fortifies everyone including people with no benefit and, at high doses, theoretical risk. It also requires a food supply centralized enough to mandate fortification at the mill or manufacturer, a lever many fragmented food systems don't have.
what came after
The US mandate became a template followed by more than 80 countries, and folic acid fortification is now credited with preventing tens of thousands of neural tube defects worldwide since 1998.
