Nutrition education aimed at young mothers barely moved child feeding, because mothers didn't decide what infants ate
背景
International child-nutrition programs had spent decades running education sessions for young mothers in West Africa, teaching exclusive breastfeeding and proper weaning foods, and getting persistently disappointing uptake. Public health researcher Judi Aubel's fieldwork in Senegal's Serer villages found the reason: in the extended-family compounds where most rural Senegalese live, a young mother rarely controls her own child's diet. That authority sits with her mother-in-law and the compound's senior women, who by tradition oversee pregnancy, birth and infant feeding and whose word a young wife does not override.
Aid programs had been training the household member with the least standing to change anything, then measuring failure and concluding that mothers needed yet more education. Aubel's Grandmother Project reasoned that if grandmothers held the actual decision rights over infant feeding, sitting them through the same lectures aimed at their daughters-in-law would keep producing the same result — the message was landing on a person with no authority to act on it.

The available playbook was to run more nutrition education for mothers, translate the materials better, add incentives for attendance, or blame low literacy — all of which kept training the household member whose feeding decisions could be, and routinely were, overruled by a senior woman who had never been in the room.

Handing the same content to the person who already holds cultural authority over the decision collapses the persuasion problem: a grandmother telling her daughter-in-law to feed colostrum carries the weight of tradition and hierarchy that no NGO poster can replicate, and because she's also the one physically present for births and early feeding, she can act on new information immediately rather than having to negotiate around someone senior to her. The fix works precisely because it stops treating the household as an undifferentiated audience and instead maps who actually decides.

It requires accurately identifying the true decision-maker, which varies by culture and can misfire if authority is more distributed or contested than assumed, and it needs the elder to be reachable and receptive to updated practices rather than defensively protective of old ones. Where household decision rights are shifting — urbanization, nuclear families, working mothers with more autonomy — targeting grandmothers alone can under-reach the people now actually in charge.

The approach reshaped how UNICEF, USAID and other agencies design maternal and child health programs across West Africa, moving many from mothers-only curricula toward household- and grandmother-inclusive models, and Aubel's peer-reviewed framework on grandmothers as 'culturally designated advisors' is now a standard reference in the field.
